Forms of payment accepted by the Health Department are cash or check, we are unable to process credit/debit card payments in the office at this time.

Food Service Permit Applicaton
|
A Food Service Permit is required for all restaurants, grocery stores, bakeries, etc. The permit is valid for one year. Home food preparation is not allowed except that which falls under the Texas Cottage Food Law. The permit fee is outlined on the application. |

Temporary Food Service Permit:
|
A Temporary Food Service Permit is required for all temporary food vendors for special events. The permit is valid for up to 14 days and costs $50 per event. Vendors now have the option to apply online.

Complete and submit application & payment online. Payment is required with credit or debit card. Currently, this is the only permit application we are able to receive online.

Nonprofit/no payment required can email application to healthdept@cor.gov. Payment must accompany application for all other vendors, submit by either mail or bring to the Health Department.

| Mobile Food Vendor Permit Application |
A Mobile Food Vendor includes hot trucks, cold trucks, and ice cream trucks. Push carts and stand alone food kiosks are not allowed. Mobile vendors are NOT ALLOWED in City parks unless permission is granted by the Parks Department.
Mobile vendors are required to obtain a solicitors permit from the Police Department before operating.

| Registered Family Home Permit Application |
A Registered Family Home Permit is required for all in-home child care providers. The permit is valid for one year.
An application may also be required by the State of Texas. Please see the Guidelines for Registered Family Homes for more information.

| Swimming Pool Permit Application |
A Public/Semi-public Swimming Pool Permit is required for all properties that have a swimming pool that is accessible to residents or the public. The permit is valid for one year. Renewals are mailed out each year May 1st. |
| Liquid Waste Hauler Permit Application |
A Liquid Waste Hauler Permit is required for all liquid waste transporters.
Trip ticket books are also required and are available at the Health Department. The permit is valid for one year.
